Discharge to Assess
Discharge to Assess is subject to change nationally, regionally and locally.
Please liaise with the iSPA, Commissioning and/or Head of Service (Promoting Independence) for the latest guidance.
All discharge to assess beds are co-ordinated via the iSPA (Integrated Single Point of Access)
Please ensure that Block Beds are all occupied before you use the Spot Bed CPLIs.
There are 20 block beds at West View Lodge for residential Rehab and Transitional support.
Additional block beds are provided across the town and include Nursing and Residential beds. The provision of these beds is co-ordinated by the Commissioning Team.
Discharge to Assess beds are available for UP TO 6 weeks. This needs to be clearly communicated to the Service User and/or their family/carer.
Anyone residing in a Discharge to Assess bed needs to be considered for further Health funding at week 2. If support is required beyond week 4 then this needs to be changed on Controcc to Post D2A unless they are awaiting a DST. This is because of a change in funding streams.
Review – If the outcome of a review of a person in a Discharge to Assess block bed is that their needs can best be met in 24-hour care, then they can only remain in that home if it is appropriate and there is another vacancy. Staff should liaise with iSPA and Commissioning to discuss options if necessary.
Discharge to Assess beds must be vacated as soon as possible to facilitate further flow from the hospital.
Discharge to Assess beds are audited weekly by the Head of Service.
